Account Closing and Loss Reporting

Certificate Loss Reporting (Written or Verbal)
1. If the certificate of deposit (or passbook) is lost, the customer needs to file a written application for loss reporting to the outlet where the account was opened, by presenting his/her valid identity certificate and some account details like his/her name and account number.
2. Loss of the certificate of deposit (or passbook) may be reported verbally in special circumstances that prevent the customer to report loss in writing, but written loss reporting should be made subsequently within five days. Otherwise, the verbal loss reporting will become invalid.
3. Loss reporting does not apply to the bearer certificate of deposit (or passbook).
4. If loss of the certificate of deposit (or passbook) is reported by an authorized representative of the depositor, valid identity certificates of the depositor and his/her representative should be presented. Seven days after loss reporting, the customer may request a new certificate of deposit (or passbook) or withdraw deposit by presenting the Application Form for Loss Reporting and his/her valid identity certificate. This procedure must be done by the depositor in person.
Application Channels
Verbal loss reporting may be made by telephone, mail or telegraph. Written loss reporting must be done at the outlet of ABC where the account was opened; valid identity certificate is required.

Password Loss Reporting
1. The customer needs to file a written application for password loss reporting in person by presenting his/her valid identity certificate and the certificate of deposit or (passbook). If loss of password is reported by an authorized representative of the depositor, the certificate of deposit (or passbook) and valid identity certificates of the depositor and his/her representative should be presented.
2. Password resetting must be done by the depositor in person.

Seal Loss Reporting
The customer needs to file a written application for seal loss reporting to his/her branch where the account was opened, by presenting his/her valid identity certificate and the certificate of deposit (or passbook).

Cancellation of Loss Reporting
If the customer requests cancellation of loss reporting of the certificate of deposit (or passbook) or password and the loss was reported in writing, the customer needs to present his/her valid identity certificate, Application Form for Loss Reporting and the reportedly lost certificate of deposit (or passbook) to the outlet to which the loss was initially reported.

Account Closing
To close a personal deposit account, the certificate of deposit (or passbook) must be presented, and relevant information will be checked against the specified withdrawal control instruments.
